Education Session: Quality in the Nursing Home

Of late nursing homes have faced an alphabet soup of quality initiatives. At the same time, concerns of safety have been raised publically through the Office of Inspector General’s report on Adverse Events in the SNF. This session will focus on three clinical initiatives: Reducing Hospital Admissions; Reducing Antipsychotic medication use and improving person-centered dementia care; and reducing health care acquired infections with a focus on C.Diff. We will look at Advancing Excellence tools as a core base, with QAPI as the frame work, all while looking to develop the person-centered culture of safety that we and the residents/families we serve are looking for.

Objectives:

1) Describe the 5 elements of QAPI and how they relate to the 3 clinical initiatives presented

2) Understand how data tracking is essential to any quality improvement initiative

3) State 3 components of a “person-centered culture of safety”
